Doctor Appointment One-Pager
Turns your notes into one page for a doctor: a dated timeline, the facts they ask for and three questions.
For: Anyone seeing a doctor about a health problem they want to explain well
You get: One page to hand over or read from, and a two-sentence opening for the first minute
Different: Builds a timeline from first change to today, fills a fixed checklist of what clinicians ask per symptom, and ranks three questions by what you need settled.
When to use it
Someone has an appointment coming up with a doctor, a nurse or another clinician about a problem that has been going on for a while. They have notes in their phone, memories of when it started, a list of things they have tried, and worries. Appointments are short, and it is easy to leave having forgotten to mention something or to ask what you meant to. This Move turns what the user knows into one page they can hand over or read from.
One input (the user's notes), one output (the page and an opening), in one sitting.
It works for a first appointment about something new, a follow-up when things have changed, or an appointment for someone the user cares for, with that person's agreement.
Not for: deciding whether something is an emergency, working out what the problem is, or deciding on treatment. Those are the clinician's job.
What it asks you
Ask for everything in one numbered message.
- Who the appointment is for (you, or someone you care for), and with whom (family doctor, specialist, nurse, other).
- The main problem in your own words, and anything else you want to mention.
- When it started, and how it has changed since: dates or rough times ("early September", "about three weeks ago") and what was different.
- Your notes: anything you have written down, pasted as they are.
- What you have tried, and what happened: medicines bought or prescribed, changes you made, other appointments.
- What worries you most, and what you most want to leave the appointment knowing.
Work from what comes back. If a date is vague, keep it vague and say so; never pin a date the user did not give. Never add a symptom, a measurement or a detail the user did not mention.
What comes back
Plain text. Headings in capitals. Short lines.
FOR Who, which appointment, date if given.
OPENING (TWO SENTENCES) What to say in the first minute.
TIMELINE Date or time, then what changed. Oldest first, today last.
MAIN PROBLEM The checklist, one line per item.
OTHER THINGS TO MENTION One line each.
TRIED SO FAR What, when, what happened.
MEDICINES, ALLERGIES, BACKGROUND Only what the user gave.
MY THREE QUESTIONS Ranked, one line each. Then "Also, if there is time:" with any others.
AFTER THE APPOINTMENT Three blank lines to fill in: what the clinician thinks, what happens next, when to come back.
FLAGS Anything from Refuse or flag that applies. "None" if none.
Next: one line naming the single most useful step.
What is in the download
- 00_START_HERE.pdf: the two-minute set-up for each AI.
- 1 - Skill:
doctor-appointment-one-pager.zip, one upload to Claude, ChatGPT, Gemini, Grok or Microsoft 365 Copilot. It switches on by itself when this job comes up. - 2 - Text file for any AI: the same Move as one
.txt. Attach it to any chat, on any plan, and type "Follow this file".
It drafts, you read and send. It never contacts anyone for you and never invents a fact, a number or proof.
A Move is a single play. The whole function in a box is an agent pack.
Questions
What is a Move?
One task done with AI that most people do not know is possible, at work or at home, packaged as one skill. One input, one result, one sitting. It is not a prompt tip and not a whole agent pack.
Why is it free for 48 hours?
Every new Move is free for its first 48 hours so you can try it and vote. After that it is $9 on its own, or part of the Lifetime Pass.
What does the Lifetime Pass include?
Every Move on the board today in one download, and every Move added later, which appear on your order page as they land. Paid once. The price rises as the board grows.
How does the ranking work?
Votes, with recent votes counting more, on top of the rank each Move launched with. Counts show from 10 votes. Nothing is seeded.
Does it send anything for me?
No. It writes and you read and send. It never invents a fact, a number or proof.
